Limited Availability on Tax Forms Locally
The Ozark Regional Library-Fredericktown and the Madison County Extension Office did not receive Federal or State tax forms this year.

The Fredericktown Post Office does have a very limited amount (about 50) of the following Federal tax forms: 1040, 1040EZ, 1040 A, Earned Income Credit and a few instruction booklets. The post office does not carry state forms.

For Federal forms the number is Toll-Free; 1-800-829-1040 Hours of Operation: Monday – Friday, 7:00 a.m. – 10:00 p.m. and press 1 to request forms
