Madison County Receives FEMA Declaration
President Obama signed a Federal Disaster Declaration on June 19, 2009 covering the storm of May 8 -16, 2009.

Individuals, families and businesses that were impacted by the storm may call 1-800-621-(FEMA) 3362. The FEMA website also provided on-line registration;

www.disasterassistance.gov

To apply on-line or over the phone, you will need a pen and paper and have the following ready:

Social Security number.

Current and pre-disaster address.

Telephone number you can be reached at.

Insurance information.

Total household annual income.

Bank account and routing number if you want the disaster funds transferred directly to your bank account.

A detailed, itemized description of your losses.

The Madison County information line will play the recorded information as well at 573-783-4627.

Should you need assistance in making an application you may call the health department at 783-2747 or the Madison County Extension Office at 783-3303.
