West County Kindergarten Screenings

Kindergarten enrollment and screenings for children who will enter kindergarten in the 2013-14 school year and reside in the West County R-IV School District will be held from 10 a.m. to 6 p.m. April 15 and from 8 a.m. 3 p.m. April 16 at the Frankclay Nazarene Church. Children must be 5 years of age before Aug. 1, 2013.

Parents or legal guardians must provide all of the following information on the day of enrollment: Up-to-date immunization records; child’s state birth certificate; Social Security card; proof of residence (such as property tax statement or utility bill); Medicaid number; D.C.N. number (case number) if applying for free or reduced lunches.

A certificate form the hospital will not be accepted. Missouri birth certificates may be obtained from the St. Francois County Health Department or from the Missouri Division of Health.
