New dates and times for Free Sports Physicals

Parkland Health Center in Farmington is offering free sports physicals to all area school athletes in grades 7-12 with two options available (see dates and time below). It is strongly recommended that parents/guardians attend with the student to help verify medical history.

Each student MUST have an MSHSAA form completed and signed BEFORE being seen. Forms can be downloaded at either parklandhealthcenter.org or fhsknights.com or at mshsaa.org.

OPTION 1: July 29 & 30, 6-8 p.m.

No appointment is needed. Physicals will be given at Midwest Convenient Care, 507 Pine St. in Farmington, ph. 573-454-2466.

OPTION 2: July 21-24 and 27-31, 5-6 p.m.

Must call 573-756-6751 to make an appointment, select option 1 to arrange time and provider. Physicals will be given at Medical Arts Clinic, 1103 W. Liberty St. in Farmington.
