Advanced Directive event to be held Wednesday
Preferred Hospice and the Bismarck Senior Center are teaming up to bring an Advanced Directive and Notary event Wednesday from 10:30 a.m. to 12:30 p.m. at the Bismarck Senior Center, located at 922 Center St, Bismarck.
Refreshments and needed forms will be provided. The event is open to the public
This is a chance for anyone in the region that is interested to take action on this important health care issue for themselves or their loved ones. Advanced Directives give you the legal power to name your Durable Power of Attorney for Health Care Choices and allows you to make Health Care Choice Directives should you ever become physically or mentally unable to communicate your desired medical care choices. Your family and health care team will better understand your preferences if you express them in writing.
The State of Missouri Durable Power of Attorney for Health Care Choices and Health Care Choices Directive forms will be available for you at the event. Event organizers will be there to explain the documents and help attendees complete them as needed. Attendees can also bring the Advanced Directive forms already filled out. A public notary and witness will notarize the documents for you whether you fill them out in advance or fill them out at the event.
