The State of Missouri to the Unknown Father of Helana Jade Crader and Biological Mother, Wendy Lee DeClue:

You are hereby notified that an action has been commenced against you in the Circuit Court for the County of St. Francois, Missouri, the object and general nature which is a Petition for Adoption.

The name of the Petitioner to said action is the Sharon Merritt and the name and address of the attorney for Petitioner is: Seth A. Pegram, 101 East Columbia Street, Farmington, Missouri 63640.

You are further notified that, unless you file an answer or other pleading or shall otherwise appear and defend against the aforesaid Petition within 45 days after the 8th day of January, 2020, judgment by default will be rendered against you.
