THE STATE OF MISSOURI TO RESPONDENT, MICHAEL HAFERKAMP, JR., the father of Ariella Haferkamp, Female, DOB: 01/19/2017

You are hereby notified that an action has been commenced against you in the Circuit Court, Juvenile Division for the 24th Circuit, St. Francois County, State of Missouri, the object and general nature of which is Petition for Termination of Parental Rights to the above-mentioned child under Chapter 211 of the Missouri Revised Statutes.

The names of all parties to said action are stated above in the caption hereof and the name and

address of the attorney for the Petitioner is Nathaniel J. Bollinger of Schnapp, Fall, Silvey, Reid & Bollinger, LLC, 135 East Main Street, P. 0. Box 151, Fredericktown, MO 63645-0151 (Telephone Number is 573-783-7212).

You are notified that, unless you file an Answer or other pleading or shall otherwise appear and defend against the aforesaid Petition or other pleading, within forty-five (45) days after the 14th day of February, 2019, judgment by default will be rendered against you and your parental rights to the above-named child will be terminated.

Further, YOU ARE HEREBY NOTIFIED that you have the right to legal counsel in this matter and if you are financially unable to employ legal counsel to represent you in this matter, the Court will, upon your request, appoint counsel to represent you free of charge.
